This is a piece for misophonia, a condition in which a person can have an extreme reaction to specific sounds. For example, pencil tapping can cause a person with misophonia to go into an intense rage.
I think you are really good at getting across your concept despite most people not knowing what the specific names of many mental ailments are. Just by looking at this image I was able to tell that it had to do with a fear/sensitivity to sound.
I totally agree with Liz on this. I, myself, don't know what misophonia is but seeing that you portrayed repetition of the ears pulsing on the subject's face helps me understand that they are sensitive to sound and noise. It is easily seen that the subject is irritated as well.
I am so impressed with this piece! As Liz and Cameron said, you did a good job in portraying fear and the disorder itself. I think maybe it would be cool to add an eerie grey spotlight over the right side of the face to highlight one of the ears.
